The Challenge
Care management across 11 sites — fragmented, paper-heavy, and difficult to report on
Running a senior care programme at scale means managing a high volume of complex, interdependent workflows — client intake and assessment, care planning, rehabilitation, activities coordination, finance, and government reporting. Across nine Sparkle Care centres, PCF was managing these workflows without a unified system. Client records existed at the centre level. Assessment forms were paper-based. Finance reconciliation was done manually. Government reporting to the Bright and ILTC portals required staff to extract and submit data by hand.
The volume and variety of care assessments in a senior care setting compounds the problem. A single client may require an intake assessment, a functional assessment, a rehabilitation plan, an Individualised Care Plan (ICP), and multiple review forms over the course of their care. Without digital tools, each of these is a separate paper process — created, stored, and reviewed independently, with no automatic link to the client record or to the finance and reporting systems downstream.
For PCF, the goal was not just to digitise what existed — it was to build a system that could support the full clinical and operational complexity of multi-site senior care, without adding burden to frontline staff.
The Solution
An end-to-end platform built around how senior care actually operates
WerkDone built the PCF Senior Care Management System as a holistic, centralised platform — one system of record across all nine Sparkle Care centres. Every client record is unified: intake, care history, assessment results, ICP status, and care notes are all held in a single, searchable profile accessible to authorised staff across the network.
The platform includes a library of over 60 digital assessment forms built specifically for senior care settings — covering client intake, functional and cognitive assessments, rehabilitation tracking, ICP documentation, and more. These forms are embedded directly in the client record, eliminating paper handling and connecting assessment outcomes to care plans and downstream reporting automatically.
Finance is fully integrated. Automated finance functions handle fee calculations, subsidy tracking, and billing across all nine centres — replacing the manual reconciliation that previously consumed significant staff time each month. Alongside the finance module, WerkDone built direct integrations with the Bright and ILTC government portals, eliminating manual data submission for funding and compliance reporting. Medical device connectivity was also included, allowing clinical measurements to flow directly into the platform without re-entry.
The result is a platform that covers the full care journey — from first assessment to ongoing care management, from activities coordination to finance and government reporting — without the handoffs and reconciliation overhead that came before.
